California made history — and most families still don't know it.
On January 1, 2023, California became one of the first states in the nation to add doula services as a covered preventive benefit under Medi-Cal. The benefit was years in the making — driven by community-based doulas, birth justice advocates, and the overwhelming evidence that doula support improves maternal outcomes, particularly for Black and Indigenous families who face the highest rates of maternal mortality and complications in this state.
Medi-Cal pays for more than 40 percent of all births in California. That means hundreds of thousands of families who deliver each year in Southern California are potentially eligible for covered doula support — and most of them have no idea. What Medi-Cal's Doula Benefit Covers
California's Medi-Cal doula benefit is among the most comprehensive state doula benefits in the country. The California DHCS issued a statewide standing recommendation that all eligible Medi-Cal members benefit from doula services — meaning no separate referral from your OB or midwife is required to begin. Here's exactly what's included:
Initial Visit

First meeting with your doula — discuss your birth preferences, build your relationship, and begin preparing for labor together.
Prenatal & Postpartum
Up to 8 Visits
Eight additional visits in any combination of prenatal preparation and in-home postpartum support — used however your family needs them most.
Labor Support
Continuous Birth Support
Your doula present throughout active labor and delivery — including support for births ending in miscarriage, stillbirth, or abortion.
Extended Postpartum
2 Visits — 3 Hours Each
Two extended postpartum visits for in-depth feeding support, newborn care guidance, and emotional check-ins at home. No additional recommendation required.
Additional Visits
Up to 9 More Visits
With a second recommendation from your provider, up to nine additional postpartum visits can be added for families who need more support.
Eligibility Window
Up to 12 Months
Doula services are available throughout your pregnancy and for up to 12 months after the end of your pregnancy — even if your baby has already been born.

Most Medi-Cal members in Southern California receive their benefits through a managed care plan rather than fee-for-service Medi-Cal. The core doula benefit is the same across all plans — but enrollment, billing, and some member resources vary by plan. SoCal Doulas is contracted with the major Medi-Cal managed care plans serving Southern California families.

Free Through Medi-Cal
Medi-Cal covers a free breast pump for eligible members. Ask your OB or midwife for a prescription referral, then contact your managed care plan to order through an approved DME supplier who ships directly to your home. Order in your third trimester. If you need a pump sooner, WIC can provide a loaner pump while your plan order processes.

Do I need a referral from my doctor to use the Medi-Cal doula benefit?
No. The California Department of Health Care Services issued a statewide standing recommendation that all Medi-Cal members who are pregnant or were pregnant within the past year benefit from doula services. This standing order fulfills the referral requirement — you do not need a separate written referral from your OB or midwife to begin. SoCal Doulas handles all enrollment documentation as part of starting services with you. Some managed care plans may have slightly different processes — we confirm your specific plan's requirements during your free verification.
My Medi-Cal is through a managed care plan — does that change anything?
Most Medi-Cal members in Southern California receive benefits through a managed care plan such as IEHP, Community Health Group, Blue Shield Promise, Health Net, or Kaiser. My baby has already been born — can I still use my Medi-Cal doula benefit?
Yes — and this is one of the most underused parts of the benefit. Your Medi-Cal doula benefit is available for up to 12 months after the end of your pregnancy. If you didn't use prenatal visits, those allocations can shift to postpartum support. With a second provider recommendation, up to nine additional postpartum visits can also be added. What if I lose my Medi-Cal coverage during or after pregnancy?
California expanded postpartum Medi-Cal coverage so that qualifying members remain covered for 12 months after giving birth regardless of income changes, citizenship, or immigration status. Can I use more than one doula through Medi-Cal?
Yes — Medi-Cal allows members to use more than one doula during a single pregnancy and postpartum period. However, the total number of visits is per pregnancy — not per doula. Only one doula may bill for labor and delivery support, so you'll want to designate which doula will attend your birth. We can help you think through the best approach for your family's needs.
Does Medi-Cal cover doula support for a VBAC or cesarean birth?
Yes. Medi-Cal's doula benefit applies to all birth types — vaginal, VBAC, and cesarean. If you are planning a vaginal birth after cesarean, doula support can be particularly meaningful for the additional emotional preparation and confidence-building the VBAC journey often requires. If you are planning a repeat cesarean, your doula supports you before surgery, during as hospital policy allows, and through your immediate recovery. The benefit is the same regardless of how your birth unfolds.
