Blue Shield Promise Doula Benefit
A health equity commitment — rated 4 out of 5 by NCQA.
Blue Shield of California Promise Health Plan serves approximately 580,000 Medi-Cal members across Los Angeles and San Diego counties. Rated 4 out of 5 by the National Committee for Quality Assurance in their 2025 Medicaid Health Plan Ratings — and holding NCQA Health Equity Accreditation — Blue Shield Promise has built one of the most comprehensive maternal health programs in the state.
Blue Shield Promise has specifically prioritized expanding doula access to Black and African American families, who face disproportionately high rates of maternal mortality and complications in California. That commitment to health equity is part of why Tayna and Ifeanyi pursued credentialing with Blue Shield Promise — because the families this plan serves deserve the same quality of professional doula support as anyone else, and Blue Shield Promise is actively working to make that possible.
To get started or ask questions about your benefit, call the Blue Shield Promise Maternity Care Management team at (888) 802-4410 (TTY: 711), Monday through Friday 8am to 5pm Pacific. They can provide your doula recommendation, enroll you in the free Maternity Program, and connect you with community resources — all at no extra cost.
"Blue Shield Promise covered our doula completely. The Maternity Care Management team helped us get started quickly — and our doula was there for every prenatal visit, our entire labor, and came to our home twice after our son was born."

What Blue Shield Promise's Doula Benefit Covers
Blue Shield Promise follows the California statewide standing recommendation, meaning most eligible members can begin doula services without a separate referral from their OB. The Maternity Care Management team can also provide the recommendation directly if needed — making the process straightforward for members. Here's exactly what's included:
Initial Visit
1 Prenatal Visit
First meeting with your doula to connect, discuss birth preferences, and begin your working relationship before labor.
Prenatal & Postpartum
Up to 8 Visits
Eight additional visits in any combination of prenatal preparation and in-home postpartum support — used how your family needs them most.
Labor Support
Continuous Birth Support
Your doula with you throughout active labor and delivery — including support for births ending in miscarriage, stillbirth, or abortion.
Extended Postpartum
2 Visits — 3 Hours Each
Two extended postpartum visits in addition to the standard visits — for in-depth feeding support, newborn care, and emotional recovery check-ins.
Additional Visits
Up to 9 More Visits
With a second recommendation from your PCP or the Maternity Care Management team, up to nine additional postpartum visits can be added for families who need more support.
Eligibility Window
Up to 12 Months
Services available throughout pregnancy and for up to 12 months after the end of your pregnancy — including postpartum-only support.

Common Questions About Blue Shield Promise Doula Coverage
How do I contact the Blue Shield Promise Maternity Care Management team?
Call (888) 802-4410 (TTY: 711), available Monday through Friday 8am to 5pm Pacific. The Maternity Care Management team handles doula recommendations, Maternity Program enrollment, community resource referrals, breast pump connections, and care coordination throughout your pregnancy and up to 3 months postpartum. This team is one of the most valuable resources you have as a Blue Shield Promise member — don't hesitate to call them early in your pregnancy.
Does Blue Shield Promise require a referral to start doula services?
Blue Shield Promise requires a written recommendation from a licensed provider to begin doula services. However, the California DHCS standing recommendation means that in most cases this is handled automatically for eligible Medi-Cal members. The Maternity Care Management team can also provide the recommendation directly — so you don't necessarily need to go through your OB first. Call (888) 802-4410 and they will walk you through the process.
What is the Promising Start program and am I eligible?
Promising Start is a Blue Shield Promise initiative launched in November 2025 specifically for members in San Diego County. It provides a newborn supply kit — diapers, breastfeeding pads, and diaper-rash cream — delivered near your due date, plus Amazon vouchers for additional baby essentials in your first year. To enroll, call the Maternity Care Management team at (888) 802-4410 starting in your second trimester. The program is currently available in San Diego County only.
How do I get a breast pump through Blue Shield Promise?
Call the Maternity Care Management team at (888) 802-4410 and ask to be connected with a breast pump referral. They will connect you with an approved DME supplier who ships directly to your home. Order in your third trimester so the pump arrives before your baby. If you need a pump more urgently, WIC at 1-888-942-9675 can often provide a loaner pump while your plan order processes.
What does Blue Shield Promise cover for postpartum depression?
Blue Shield Promise has a standalone Maternal Mental Health Program — one of the more comprehensive PPD resources available through a Medi-Cal plan in California. All prenatal and postpartum providers are required to screen members for perinatal depression. If you screen positive or are struggling after birth, your provider or the Maternity Care Management team can provide a direct behavioral health referral. The National Maternal Mental Health Hotline is also available 24/7 at 1-833-852-6262 for immediate free support while you wait for a referral appointment.
Can I use my Blue Shield Promise doula benefit if my baby is already born?
Yes — your Blue Shield Promise doula benefit is available for up to 12 months after the end of your pregnancy. If you've recently delivered and haven't yet used your benefit, you may still qualify for covered in-home postpartum visits. With a second recommendation, up to 9 additional postpartum visits can also be added. Complete our free verification to confirm your current eligibility even if your baby has already arrived.
